He was 82 and he said yes to aligners in about four seconds


A doctor on our call had a patient walk in on a Monday with a broken front tooth.

Not a new problem. She had bonded that lateral before. More than once.

His canine was crashing into it every time he closed.

So the bonding kept coming off, and this time the tooth itself fractured.

He is 82.

She gave him two options.

She could crown the lower tooth that was doing the damage, and warned him that might turn into a root canal.

Or she could move it out of the way with aligners and make room for the lateral.

He said yes to the aligners almost immediately.

Here is the part that matters.

She told me she would never have brought up aligners with him on her own.

He has crowding in his lower front teeth, but she would not have pushed a patient his age into treatment.

He had a reason she did not know about.

He was tired of the same tooth breaking.

We decide who is a candidate before we ask them what they want.

She is not unusual.

Another doctor on the same call is finishing an aligner case on a 73-year-old with a perio history right now.

Eighteen months of treatment.

She gets her retainers this week and the result is beautiful.

Old is not a contraindication. Unstable is.

If they are periodontally stable and something in their mouth is breaking because of how their teeth hit, they are a candidate.

Their birth year is not the screen.

Give them the option and let them tell you no. Stop saying it for them.

Knowing which cases are safe to take and which ones to refer is most of the confidence gap.
