Why harder cases won’t grow your aligner practice


I used to think growth meant taking on harder cases.

That is what I saw other dentists doing with implants.

They would start with a single tooth, then graduate to multiple teeth, and eventually full arch cases. Complexity stacked, and so did their confidence.

So I assumed aligners worked the same way.

But it did not take long to realize they don’t.

Here is what I noticed.

On Instagram, the dentists posting complex aligner cases were not celebrating at the finish line.

They were relieved.

More complexity means more refinements.

More refinements means more time.

More time means less profitability.

And when you are charging a flat fee, that math does not work.

I learned this the hard way.

Early in my career, I took on a tough case and ended up with a posterior open bite.

I had no idea how it even happened.

Thankfully, I had a mentor to help me troubleshoot it. Without them, I would have been stuck, miserable, and embarrassed.

That experience scared me away from aligners until the next patient came in.

A simple case.

Totally different vibe.

We were laughing, talking about life, zero stress.

That is when it clicked: I did not need more complexity. I needed more efficiency.

Efficiency is the cheat code.

Instead of chasing harder cases, I doubled down on predictable ones.

We built systems, streamlined workflows, and made the “easy” cases incredibly efficient.

Because we charged a flat fee, every hour saved meant higher profit and less stress.

The bottom line:

Growth in aligners does not come from proving you can handle the toughest case.

It comes from making the simple ones predictable, profitable, and scalable.
