Stop asking for all the money upfront

Hey Friends,

You don't need all the money upfront to do aligners.

When is the last time you paid for something outright?

Nowadays everyone is either on a monthly subscription or some type of payment plan where they can stretch out payments.

It makes sense.

Why tie up your cash if you don't need to?

Also - why create a barrier where one is not needed?

What if you could let your patients go home and "think about it" or "talk to their spouse" and still get more patients to start treatment just by... asking for less money?

That is what the EZ start method is.

After you discussed aligner therapy with them and they are onboard and want to start...your team reviews the different payment options with them

Then the patient will either choose which one they want or tell you something like

"I need to talk to my spouse about it first"

Then you acknowledge that and ask for $250 to secure their spot to get started with treatment.

Let them know they have 4-5 weeks to discuss with their spouse and they can let you know which payment option they'd like to go with at the delivery appointment.

They would sign an agreement and carry on.

This may sound foreign to you.

But it's what most of the top aligner offices are doing to increase their case acceptance - and it's working.

Patients want the treatment - most of the time they fall off when they go home to think about it without putting money down because they just forget.

When they put some skin in the game they are less likely to forget and will actually have the conversation.

Give it a try and let me know how it works for you.

Straight Talk

Modern aligner success isn’t just about offering clear aligners—it’s about mastering them.

That means being able to confidently diagnose, modify treatment plans, troubleshoot when things don’t go as expected, and build trust with patients.

But here’s the problem: too many providers are skipping these steps, relying on outsourced ortho solutions, thinking it’ll save them time.

It doesn’t.

If your doctors don’t understand the fundamentals, you’re not making their lives easier—you’re setting them up for failure.

Outsourcing can only take you so far.

At some point, you or your team will have to answer patient questions, adjust a treatment plan, or problem-solve when a case isn’t tracking.

If you don’t have the confidence to do that, aligners will always feel like a headache instead of a predictable, profitable part of your practice.

The solution?

Build confidence.

Build understanding.

When you invest in the fundamentals, success follows.

Not just for your patients, but for your team and your bottom line.

Your Friend,

Dr. Avi

When patients say no to aligners, it’s rarely about money. It’s about trust. Early on, I quoted fees and hoped they’d say yes. But once I started asking better questions and explaining why it mattered, things changed. That's when most of my consults ended with a yes. Try these two shifts to improve your acceptance rate: Ask better questions. Instead of “do you want to straighten your teeth?” ask: “Has anyone ever mentioned your teeth are in an unhealthy position?” Frame the benefits around...

Most docs try to get aligners going on their own. Then feel frustrated when the team doesn’t support it. I was there too. I tried to do everything myself — scanning, educating, managing trays. My assistant was confused, my front desk forgot follow-ups, and cases stalled. When I finally trained my team the right way, everything changed. Here’s how to get your team on board fast: Start with a 15-min huddle: Explain why aligners matter (not just for you — but for patients) Front desk = follow-up...

Every dentist has one fear when starting aligners: “What if this goes wrong?” I remember my first case with rotations and black triangles. It didn’t go as planned. The patient expected perfection. I expected simplicity. We both left disappointed. Then I changed one thing: the way I set expectations. Now I use one simple script at every consult to align goals and avoid future surprises. Here’s the script: “We’re going to make a big difference in your smile — and sometimes, the plan needs to be...
