Stop Overcomplicating Your Practice—Start with the Basics (Nov 5)

I see a lot of dentists think they need to invest tens of thousands of dollars learning full-mouth rehab, veneers, or advanced procedures, but they can’t even confidently perform straightforward treatments yet.

Here’s the deal—learn the basics first, then skill up from there.

It’s also important to evaluate if your practice is even structured to implement those procedures.

For example, if you practice in an area where most of your patients are partially or completely edentulous, you should probably invest in implants and overdentures—not veneers, cosmetics, or aligners.

But for most practices, aligners are a no-brainer.

It’s plastic moving teeth—don’t overcomplicate it.

Get the right education, start offering aligners, and watch your practice grow.

The ROI on learning aligners is insane.

Plus, once those patients trust you with their aligners, they’ll be coming back for more treatment.

Aligners are comprehensive, and if you’re not offering them, you’re missing out on the low-hanging fruit.

Straight Talk

If you’re just starting out with clear aligners, knowing what to look for in each case can save you a ton of headaches—and get you much better results. Here’s what you need to keep an eye on.

First up: unnecessary rotations, especially on premolars. Clear aligner software will always try to line everything up perfectly in the arch, but that’s not always what’s best for your patient. If the occlusion is stable and rotating that premolar doesn’t improve the bite, skip it. Not every tooth needs to be moved just because the software says so. Less movement can mean fewer trays and faster treatment.

Speaking of trays—if you’ve got a case that requires 40 trays or more, that’s a red flag. It’s either an extremely difficult case (which I don’t recommend tackling as a beginner) or something’s off in the setup. Check the plan using the checklist I’ve shared with you. There’s almost always something you can tweak to bring that tray count down to a more reasonable number.

Now, here’s another big one: jumps in the ClinCheck. Sometimes when you play out the ClinCheck, you’ll see a jump or a collapse in the movement at the end. This isn’t realistic or accurate and can mess with your results. Ask the techs to remove that jump for a smoother, more predictable outcome.

If you’re working with a crossbite, there’s something you need to know. During the stages where the bite is being “jumped,” tell the patient to wear their trays while eating. This is important. It keeps the movement on track and reduces discomfort. You can pinpoint which trays are doing the work on the crossbite because it’ll look like the teeth are moving through each other—that’s when they need to wear the trays 24/7.

And lastly, if you see “roundtripping” in the setup—where a tooth moves out of alignment only to come back later—tell the techs to remove it. Roundtripping adds unnecessary complexity and tray time, and you don’t need that.

By paying attention to these details, you’ll make your clear aligner cases smoother, more predictable, and ultimately more successful.
